Admissions Procedures

When arriving, we want to make sure we have all the information we need to help you be comfortable in your new home.

When you arrive, our medical staff will need full access to your medical information so they can provide the best treatment and care services. These items are required:

• Medical record (this may be sent from the family doctor)
• Health insurance card
• Prescription drug insurance card (if applicable)
• Social insurance number
• A list of medical prescriptions
• Enough medication for 48 hours
• Contact information of respondents and family members

Also recommended to bring when you come to stay at the residence:

• Necessary items for personal hygiene
• Necessary clothing items
• Television or radio, if desired
• All necessary prostheses for the resident’s well-being (if applicable)
• Money for petty cash

Our clinical department will meet with you and begin the discovery process to determine your specific needs. That way, we will have everything ready when you move in.

As your healthcare needs change, your care plan changes to precisely meet your needs. As your health improves, your health care services change. Should you require the services of a licensed nurse, we provide that as well.

We take your health care needs very seriously. Therefore, every three months (or more frequently when needed), we hold a Family Connection Conference with you and your loved ones to thoroughly discuss your health needs and to review or adjust your services.
